Steps To Follow

Step 1

Call Northwest Bergen Regional Health Commission at (201) 445-7217 to determine which type of application you need. NWBRHC processes Health Department Licenses for Midland Park and Waldwick only. For all other towns under NWBRHC jurisdiction, you must complete a Health Department License application with the borough or town in which you operate. Click here for your municipality.

Download and complete the appropriate application below. 

Step 3

Bring the application and a check, money order, or cash to NWBRHC at 20 West Prospect Street, Waldwick, NJ 07463. Sorry, we do not accept credit cards.
